2024-2025 Annual K-12 Health Assessment and Immunization Report

General Instructions

Please take the time to review this landing page. Then, download and read all of the linked worksheets and use them for references as you are completing the online report. There are blank worksheets and worksheet instructions for the Health Assessment and the Immunization Report.

Once you have prepared your responses and are ready to begin that process, scroll to the bottom of this page and click on the NEXT button to begin the online report.

Note that after entering school and contact information, private schools will be automatically directed to the Immunization portion of the report they are required to submit, unless they choose to complete the Health Assessment.
Other schools will complete the Health Assessment Report and then be redirected to the Immunization Report.


Deadline Updates due to Hurricane Helene:
Per Executive Order 319 the deadlines for student compliance with immunization and health assessment requirements and for school reporting have been extended due to the State of Emergency caused by Hurricane Helene.
The time period from the first day of school through October 31st, 2024, will not count toward the 30-day grace period for students to comply with immunization requirements or submit proof of a health assessment. The 30-day grace period will start November 1st 2024. The exclusion date is December 1st, 2024.

The deadline for schools to submit their annual report has been extended until January 15th, 2025. Please note, these reports should reflect student compliance status on the new deadline date.

If you need to edit a report that you previously submitted, please do not submit a second report. Contact Colleen Ehatt at colleen.ehatt@dhhs.nc.gov and she will assist you with your initial report.
Health Assessment Report Instructions
(Private schools are NOT subject to the Health Assessment Report requirements)

Students enrolling in a North Carolina public school (including charter and special schools) for the first time are required to present a Health Assessment Transmittal Form within 30 calendar days of their enrollment.  

Private schools are not required to submit the Health Assessment portion of this report, but must complete the Immunization Report questions. Private schools have the option to complete the Health Assessment Report for your own records, if they choose to do so. If a private school does not choose to complete the Health Assessment Report, they will be automatically directed to start on the Immunization Report for Kindergarten, 7th and 12th grades.

  • This worksheet should be used as a tool to gather the necessary information before entering it into the online report.
  • Note: Reporting the enrollment for new students, reported on the Health Assessment Report, is different from the total student enrollment, reported on the Immunization Report.

Immunization Report Instructions
(ALL schools are subject to the Immunization Report requirements)
 

You will be redirected to complete the 2024-25 Kindergarten, 7th Grade, and 12th Grade Immunization Report after completing the Health Assessment Report. Unlike the Health Assessment Report, all schools must complete the Immunization Report.

When your responses are ready, please return to this page and enter them electronically. Do NOT submit OR mail the worksheet itself.

Note that you will be asked to report the kindergarten enrollment for all students-this will be different from the kindergarten enrollment reported in the Health Assessment Report. The Immunization report will ask for the total number of kindergarten students enrolled in your school, including those repeating the grade, but the Health Assessment Report will ask for the total number of kindergarten students enrolled for the first time in an NC public school.

 
Important Notes:

FireFox and Chrome are preferred browsers for completing this online report as they are more compatible with Alchemer (formerly Survey Gizmo). It is not suggested that you use Internet Explorer to complete this online report.
