3. Applicant College Loop Role/Title *This question is required. For ex: President, Treasurer, Faculty Advisor

4. College Loop Code *This question is required. Please input your College Loop Code. Hint: It's formatted as "CLXXXXX" and can be found in HQ, the CL Codes sheet or by emailing CollegeLoops@girlswhocode.com
