Minnesota Statutes 2024, section 134.51, subdivision 5(d) directs the governing bodies of public libraries to report content challenges to the commissioner of education. Use this form to provide the information required by statute for each reported challenge.
Please submit one report for each incident. The report will be reviewed by staff at State Library Services at the Minnesota Department of Education. If you have questions about this form, please send your questions to mde.lst@state.mn.us.
