Please fill out all required fields for the DOD Fresh Fruit & Vegetable_FFAVORS User Request Form. This form is specially for sponsors who are using the DoD Fresh Fruit and Vegetable Program.

Submission Deadlines for Request Review:
Requests will be reviewed weekly on Fridays and a minimum after the 10th of each month.

Please note: This form is to be completed by the Designated Official only (Program or Ordering Contact per CNPweb). The Program operates July 1st through June 30th each year.
