BEAT Lung Cancer: Biomarker Testing Survey

Thank you for participating in the BEAT Lung Cancer: Biomarker Testing Survey. Your participation is very important in order to help us learn more about experiences people have with biomarker testing for lung cancer.

This survey is conducted by the Research and Evaluation Group (R&E Group) at Public Health Management Corporation (PHMC) in partnership with the American Lung Association (ALA).

You do not have to respond to questions that you do not want to answer, however, your responses are extremely important to us. All responses and personal health information is protected and will be kept anonymous and used solely for reporting purposes unless otherwise selected by the individual to participate in Biomarker Testing Education and Awareness education opportunities.
