Wisconsin Department of Health Services Internal Aging and Disabilities Resource Center (ADRC) Creative Concept Testing Survey

The Wisconsin Department of Health Services’ (DHS) Bureau of Aging and Disability Resources (BADR) is currently seeking feedback on potential 2025 updates to aging and disability resource center (ADRC) customer campaign materials from Wisconsin residents who have some familiarity with ADRCs or a Tribal aging and disability resource specialist (ADRS).

We are inviting customers, caregivers, family members, and other Wisconsin residents familiar with ADRCs or Tribal ADRSs to offer your input on these materials. We are hoping to learn which creative concepts (such as the addition of a tagline, a refreshed color palette, a redesigned logo, and other visual elements) resonate most with you, and why.

Your answers to this short survey will help guide the direction of our campaign. Thank you very much for sharing your thoughts with us.
