Wisconsin Inventory of Suicide Prevention Programs and Activities

The Wisconsin Department of Health Services (DHS), Division of Public Health (DPH) was awarded funds for the Centers for Disease Control and Prevention (CDC) Comprehensive Suicide Prevention (CSP) Program in 2022. One key aim of the CSP Program is to create an inventory of existing suicide prevention programs and activities occurring in Wisconsin, including those that serve two disproportionately affected populations (DAP):
  1. Rural males 25 years and older.
  2. Adolescents ages 10-19.
The purpose of this survey is to inventory all suicide prevention programs occurring in Wisconsin so that future programming can avoid duplication of existing efforts, complement or add to what is already happening, and fill gaps that may exists.

When answering the following questions, please consider any suicide prevention programs or activities supported by your organization including formal programs, interventions, or policies. Before beginning the survey, it may be best to make a list of all suicide prevention programs and activities (including those related to self-harm) currently taking place in your organization. The survey will ask questions about one program or activity at a time. This survey will take approximately 15 minutes.
