LGBTQ+ Organisations: Health System Collaborations Survey

About this Survey

Thank you for participating in our survey, the data you provide us with will help us to co-produce a set of resources intended to further resource LGBTQ+ organisations in their work with local health systems.

This survey will take around 10 minutes to complete, or a little longer depending on how detailed your answers are! Upon completing the survey you'll be entered into a prize draw. There are 2 £50 love to shop vouchers to win.

This survey is aimed at people who work, volunteer or act as a trustee for an LGBT+ group, network, project or organisation. For the purposes of this survey we use the word Group to cover all organisations, regardless of their size, their structure or how formal or informal they are.

You will need to have some basic information about your Group to hand to be able to complete this survey.

You should know that we will never share your data with third parties unless you give us permission to do so.
