This survey should only be completed if you are a student or faculty/staff member at Youngstown State University and are requesting a Late Registration Change after the last day to add a class has passed.

This survey can also be used by faculty/staff who need to alter a student's schedule based on situations such as inadequate prerequisite grades or removal from an academic program.
