Frontotemporal Dementia Caregiver Survey
You have been invited to participate in this survey as you are a caregiver of a family member or friend of a person diagnosed with frontotemporal dementia (FTD). There are additional criteria to participate in the survey which will be reviewed in the screening questions that follow. Professional caregivers who take care of individuals with FTD as a form of employment are not eligible.
The survey is being conducted by Alector, a biopharmaceutical company. Alector would like to better understand your experiences as a caregiver of an individual diagnosed with FTD, your interactions with healthcare providers, supportive services, and day-to-day caregiving challenges.
By completing this survey, you will help us understand how we might improve care and resources for people and families living with FTD. Your input is critical to inform us on how to best support caregivers.
The survey will take around 20 minutes to complete.
