The Sedgwick County Health Department would like to learn from residents and stakeholders about their thoughts, perceptions and desires about the health of their community. The information will be used for the Sedgwick County Community Health Improvement Plan. Your feedback is vital. Let your voice be heard! The survey consists of several questions. Completing this survey is voluntary and can be stopped at any time. Whether or not you complete the survey will not affect the services you receive from the Sedgwick County Health Department. Responses will be confidential and will not be identified by individual. All responses will be collected and analyzed in a group.
