The Northwest Michigan Community Health Innovation Region is implementing a Behavioral Health Initiative to strengthen behavioral health systems and promote well-being and resiliency across 10 counties in northern Michigan.
The purpose of this survey is to assess knowledge, attitudes, and awareness related to mental illness and those experiencing mental illness.
For this survey, mental illness is defined as a condition that affects a person's thinking, feeling or mood. Such conditions may affect someone's ability to relate to others and function each day. These mental illnesses may include: schizophrenia, post-traumatic stress disorder, bipolar, borderline personality disorder, attention deficit hyperactivity disorder, obsessive compulsive disorder, depression, anxiety, eating disorders, suicide, addiction and others.
At the end of the survey, you can choose to be entered into a drawing for a chance to win a $50 gift card.
