General Practice Queensland Limited ACN 123 426 111 trading as CheckUP Australia (CheckUP) has been funded by the Federal Health Department to coordinate the delivery of Outreach health services to your community.

If you provide your name and contact details they will be kept private in accordance with the Privacy Act 1988 (Cth) (Privacy Act).The CheckUP Privacy Policy is available on the CheckUP website here.
Secure data storage
Your survey answers are safely stored on the Alchemer survey platform based in Canada, the United States of America and Germany. This platform has very high standards for keeping your data safe. Please refer to the Alchemer website if you want to read their Privacy Policy.
What we use the data for
Providing feedback is entirely your decision, and will not affect your medical treatment in any way. Non-identifying survey results may be shared with funders, your health provider or other stakeholder for statistical, evaluation and quality improvement purposes.
