Code of Conduct Violation Complaint

  • A Code of Conduct Violation Complaint must be filed within 12 months after discovery of the facts supporting an alleged violation. City Code § 2-580(d)(3).
  • Please complete a separate form for each alleged violator and violation. City Code § 2-580(d)(4).
  • Pursuant to City Code § 2-580(c), complaints and information about an investigation will be kept confidential except:
    • in accordance with the Colorado Open Records Act, or
    • if the complaint warrants investigation and the City Attorney or their designee produces written findings of fact and conclusions of law to the City Council.
1. Complainant Information
  • Complaints of alleged Code of Conduct violations may be filed by any person, Council or board or commission member, City staff, City contractor or vendor, or a person whose employment gives them access to or contact with Council or the board or commission at issue. City Code § 2-580(d)(1).
  • Please provide at least one method of communication.
