Thank you for taking the 2024 Montezuma County Community Health Assessment! This survey is completely anonymous, and will be used to inform the Montezuma County Public Health Department’s 5-year public health improvement plan. This is our best opportunity to hear your voice! We realize some of these questions may seem personal, but your honesty allows us to better meet the needs of the community. Outcomes of this survey will be publicly presented in December.
