By participating, you authorize and consent to the HER Foundation, a nonprofit foundation based in the United States, and it's affiliates, partners, successors, associates and assignees ("Parties") to reproduce, modify, and utilize all content entered including images, likeness and voice through video and photographs, digital audio and video recordings,  or any other means, including color or black/white, partial or composite renditions of any media in any way or in conjunction with any other media for fundraising, education, advertising, public relations, social media, commercial, business or any other legal purpose in any medium for any domestic or international purpose and audience on the website or other internet site associated with HER Foundation, or in print format ("Acceptable Uses"). By completing this form, you agree to allow the HER Foundation and Parties world-wide, royalty-free, perpetual, non-exclusive license to copyright, distribute, broadcast, modify, publish and/or otherwise use any media and information you submit for uses previously stated.

I agree that my content, likeness, video, images or other material may be edited and used by the Parties for the press, print media, radio and internet, cable, broadcast, and exhibition or any other purpose in perpetuity throughout the world. Parties may use any and all content for purposes of promotion, reprint, education and awareness about Hyperemesis Gravidarum or the HER Foundation and its programs without limitation or liability.

I understand that I will not receive compensation for any Acceptable Use of any media, content or materials submitted by me or created from my information, images, recordings, print or any type of media and distribution worldwide.
