Sumner County wants to hear from you regarding community health needs and concerns. The following survey has been created in order to understand the state of health services in Sumner County, learn about how community members use local health services, and to gather feedback on how the local health systems can improve. The survey should take 15 to 20 minutes to complete. You must be over the age of 18 to complete the survey.
Participation in the survey is voluntary and your responses will be anonymous.
