Clean Fleet EV and EV Charging Program Incentive Redemption Form
Thank you for submitting the required information and documentation to process your Clean Fleet EV and EVI Charging Program incentive.

Please have the following information available before proceeding with this form.
  • Your application number.
  • Proof of purchase for the incentivized chargers (with date, make/model, and costs of chargers visible).
  • Proof of purchase and VINs for the incentivized BEVs (with date, make/model, and costs of BEVs visible).
  • Site photos of the completed project.
  • Make, model and serial numbers for all incentivized chargers.
  • A copy of your network provider agreement.
