Clean Fleet EV and EV Charging Program Incentive Redemption Form

Please have the following information available before proceeding with this form.
- Your application number.
- Proof of purchase for the incentivized chargers (with date, make/model, and costs of chargers visible).
- Proof of purchase and VINs for the incentivized BEVs (with date, make/model, and costs of BEVs visible).
- Site photos of the completed project.
- Make, model and serial numbers for all incentivized chargers.
- A copy of your network provider agreement.
