Caregiver's Satisfaction Survey: English

Sealing Smiles Across New Hampshire | Nashua School District

During the past school year, the Nashua School District, New Hampshire Department of Health and Human Services (NH DHHS) Oral Health Program, and Solvere Health worked together to host a free cavity prevention program at elementary and middle schools called Sealing Smiles Across New Hampshire (SSANH). The program educated students about the importance of daily flossing and brushing, screened students to detect any concerns, and applied sealants to prevent cavities if necessary.  These services were available to all students for free with no copays or deductibles.

We are now reaching out to get your feedback. Please complete this short survey to help us understand your satisfaction with the service. The survey should only take approximately 5 minutes to complete.

Please complete this survey by Wednesday, June 8, 2024.
