Thank you for your interest in this study about your community’s health needs, focusing on issues related to stroke and smoking. Your participation is completely voluntary. You may decline to take part or withdraw at any time. There are no known risks to participate in this study.

This study is conducted by the World Bank with the 🇲🇩 Ministry of Health. It should take about 20 minutes to complete.

If you complete the study, you will be entered into the chance to win a gift card for airtime worth 1,000 lei. You will receive your ticket number at the end of the survey, and will be informed if you received the winning number directly.

Your answers will be collected anonymously, encrypted in transit, stored securely, and kept confidential. They will only be seen by the research team. If at any time you have questions about the study or your data, you can write an email to embedresearch@worldbank.org.
