The purpose of this survey is to find out more about your child’s experiences in this afterschool program. The answers you provide will help make afterschool programs better for students in Arkansas. We need your honest feedback. The questions on the survey ask about what your child experienced in the afterschool activities offered at this program this school year. These activities include those that your child went to in person before school, afterschool, or on weekends and those they may have attended online. The term “afterschool” used in this survey refers to all of these types of activities.
