Thank you for your interest in completing the Mecosta County Substance Use Stigma Assessment Survey to help us better understand what healthcare providers think about substance use disorders and people experiencing substance use disorders. This survey is for any healthcare staff, mental healthcare providers, and substance use providers who treat or come in contact with individuals experiencing substance use disorder in Mecosta County.

By taking this survey, you agree that the information you provide can be used by District Health Department #10. Your responses will remain anonymous and confidential.
