Moolah for Schools 2024-2025 Application

NOTE: You will be asked for specific School Meal Program monthly data as part of your grant submission.

Please carefully complete all sections of the grant in entirety. Pay special attention to school information. Partial and/or incomplete applications will not be processed. Please submit one application per school district. Application MUST be completed by Food Service Director or District Food Service Representative (assistant director, administrative assistant, dietitian, chef, etc.).

1. MDE School District Code (numbers only) *This question is required. This is your site Agreement Number. It is located in the Coordinated Application in NexSys through MDE. If you do not have access or know your school district site Agreement Number, please visit the Education Entity Master (EEM) and enter your school district in the search bar. The Entity Code or Agreement Number should be a 5-digit number. This question requires a valid number format.
