Personal Information: The following survey asks for information so that your application for a parenting programme can be considered by Telford and Wrekin Council Family Hubs, and so that, if you are eligible, a registration code be released for you to access the programme. To do this, Telford and Wrekin Council Family Hubs need to ask you for specific information, including your name, contact details, and postcode. This information will be used to assess eligibility, provide access to the parenting programme and to enable Telford and Wrekin Council Family Hubs to understand how many parents are accessing Triple P Online and how many parents complete the course. The Survey also asks about your ethnicity, which is a form of 'special category' personal data. Providing this information is discretionary. You do not need to disclose your ethnicity to be eligible for the parenting programme. If you choose to provide Telford and Wrekin Council Family Hubs with your ethnicity, it will be used for the purposes outlined in our
Privacy Notice.
The information collected through the survey will not be available to or shared with the Triple P Group.
