Dear SFSP sponsor,
Thank you for participating in Washington’s Farm to Summer Week, July 15-19, 2024! Please complete this survey to share information about your summer program and give us feedback about WA Farm to Summer Week. It has 13 questions and will take approximately 5-10 minutes to complete.

We will use this information to promote Farm to Summer work and improve our support for Farm to Summer activities in future years. Your responses are valuable, and we thank you in advance for taking the time to provide them.

We must inform you that your responses are not confidential; however, we will not share any personal contact information including name, title, phone, and email. Sponsors that participate in Farm to SFSP Week will be celebrated on OSPI’s social media account.
