In 2023, the Cook County Health Office of Behavioral Health (OBH) was created to improve mental health and substance use services in Cook County. The OBH plans to invest $44 million from the American Rescue Plan Act of 2021 to support mental health and substance use services across agencies, programs, treatment, and care in Cook County.
The OBH is doing a countywide community needs assessment to understand the unmet mental health and substance use needs of residents of Cook County, and to identify priority topic areas for funding. This survey is part of the community needs assessment and aims to lift the voices of people like you - Cook County residents, mental health providers, and organizational leaders. You can help identify the experiences, needs, and wishes of our community.
This survey is confidential and takes 5 - 8 minutes to complete.
The OBH will summarize the data from the survey and share it with the Cook County community in a Summer 2024 report.
We thank you in advance for taking part in the survey!
