Tower Health Community Health Needs Assessment

Welcome to the Tower Health Community Health Needs Assessment Survey.

This survey will only take about 10 minutes. We will ask you questions about the health needs of you and your community. The information we get from the survey will help us:

• Identify health problems that affect the people in your community
• Better understand the needs of your community
• Work together to find solutions to address those needs

The survey is voluntary, and you do not have to take part. You can also skip any questions you do not want to answer or stop the survey at any time.
