This survey is completely anonymous and should be completed by one individual per household. Your feedback is vital to understand barriers to internet and device access, affordability, and adoption. After completing the survey, you will have a chance to win one of ten $100 gift cards, if interested. You will be asked to provide contact information which will only be used to contact you if you are a gift card winner. You can still complete the survey and not enter to win a gift card.
