Horizon Public Health is providing an opportunity for you to engage in sharing about what supports you in being healthy.
Creating Community Consulting will facilitate a discussion where they will engage in asking you about what helps you to feel well, and what barriers or conditions cause you to not feel well. Then we will brainstorm ideas for solutions and prioritize a community action plan for improved mental well-being. The information gathered will remain anonymous and will be utilized to improve strategies and services at Horizon Public Health.
Compensation is available for your participation in the form of a $50.00 check which will be mailed to you following the session. Compensation is limited on a first come first serve basis. Registration is recommended, but not required.
