The General Fraternity recognizes chapter and individual success and achievement at the General Convention each summer. Some General Fraternity awards permit chapters to receive credit for their participation in campus-based, district, regional, and third-party leadership development programs. This form should be used when a chapter or individual is seeking to submit their participation in such a program for consideration during the General Fraternity's annual awards process.

Before filling out the form, gather the following:
- Proof of Registration (confirmation email, nametag from event)
- Proof of attendance (photos from the event)
- Agenda from the program/event
- Workshop printed material
- Completion certificate (if applicable)
