General Fraternity Awards - Request for Approved Leadership Development Programs

General Information

The General Fraternity recognizes chapter and individual success and achievement at the General Convention each summer. Some General Fraternity awards permit chapters to receive credit for their participation in campus-based, district, regional, and third-party leadership development programs. This form should be used when a chapter or individual is seeking to submit their participation in such a program for consideration during the General Fraternity's annual awards process.

Read each question carefully and ensure all requested details are provided. 

Before filling out the form, gather the following:

  • Proof of Registration (confirmation email, nametag from event)
  • Proof of attendance (photos from the event)
  • Agenda from the program/event
  • Workshop printed material
  • Completion certificate (if applicable)
