Pop-up Farmers Market: Monthly Report

Pop-up Farmers Market (PUFM) reports are due by the 1st of each month, with a five-day grace period. (i.e., Jan 2024 report is due by Feb 1st, 2024). Reports are considered late by the 6th day of the month.

Agencies that fail to submit their monthly report will be suspended and will not receive their PUFM vegetable boxes until the report is submitted.

Note: Agencies that fail to submit their PUFM reports for four (4) consecutive months on time will be suspended for two (2) consecutive distributions after the month’s late reports are submitted.

Files should be titled Agency Name + PUFM Sign-in + Month and year
- i.e. PBCFB PUFM Sign-in Jan 2024
Files must be COLOR PDF copies
You can upload up to ten (10) separate files of approximately 120 pages each (50 megabytes).
