Senior Hunger Initiatives (SHI): Monthly Report


The Senior Hunger Initiatives (SHI) program consist of two components:
  • Groceries for Seniors (GFS): shelf-stable food bag
  • Produce for Seniors (PFS): fresh produce box

Senior Hunger Initiatives (SHI) reports are due by the 1st of each month, with a five-day grace period. (i.e., Jan 2024 report is due by Feb 1st, 2024). Reports are considered late by the 6th day of the month.

Agencies that fail to submit their monthly report will be suspended and will not receive their GFS bags until the report is submitted.

Note: Agencies that fail to submit their SHI reports for four (4) consecutive months on time will be suspended for two (2) consecutive distributions after the month’s late reports are submitted.

5. Distribution Report (GFS sign-in logs): (Old GFS sign-in log)
  • Order of report: GFS Sign-in sheets, organized by dates
  • Files should be titled Agency Name + GFS Sign-in + Month and year
    • i.e. PBCFB GFS Sign-in Jan 2024
  • Files must be COLOR PDF copies
  • You can upload up to ten (10) separate files of approximately 120 pages each (50 megabytes).
  • Accepted formats: png, gif, jpg, jpeg, doc, xls, docx, xlsx, and pdf

  • Files should be titled Agency Name + SHI Sign-in + Month and year
