Introduction

1. Hello! This is a survey about mental health services you received from your Community Mental Health agency in Southwest Michigan.

Southwest Michigan Behavioral Health (SWMBH) is a group that works with the places listed below. SWMBH wants to know what you think about the mental health services you or your family received. The survey asks about the people who tried to help you or your family. The survey also asks if you feel like the help you got improved things for you or your family.

What you say will only be used to make services and programs better. Your name or your answers will not be shared with anyone else.

There is room at the end of the survey to tell us more if you would like to.
