Improving Asthma in Ohio Schools Scholarship Form

Thank you for your interest in joining our efforts to improve pediatric asthma in Ohio! The Lung Association is providing targeted outreach in Ohio to recruit school nurses to deliver asthma self-management education using the American Lung Association’s Open Airways for Schools® (for ages 8-11) and Kickin’ Asthma (for ages 11 - 16) programs. 

For a limited time, you can become a trained as a certified facilitator to deliver Open Airways for Schools (OAS) and/or Kickin' Asthma (KA) at no cost to you. The training scholarship includes: the blended learning training, technical assistance with Lung Association staff, ongoing support, additional online resources and a facilitator toolkit, which includes demo devices, curriculum, and materials). Scholarships are limited and available on a first-come, first-serve basis. 

To be eligible for the Improving Asthma in Ohio scholarships, you must:
  • Serve children in a school or community setting in Ohio.
  • Complete facilitator training no later than 6/30/2024.
  • Implement at least one OAS or Kickin' Asthma program (in-person or virtually) before 12/31/2024 (minimum 3 children per program).
  • Submit a Facilitator Report Form after each program. 
  • Have students complete a confidential OAS or KA Participant Evaluation Form.
This scholarship application form should take 2-3 minutes to complete. 

In addition, applicants will be encouraged to complete the course, Stock Asthma Medication: Implementation Guidance for Schools, and will be eligible to receive a $100 stipend to purchase supplies to support implementation of your Stock Asthma Medication in Schools program.
