Ethical Issue Submission Form

Introduction

This form will be utilized to submit ethical issues to the committee for review and resolution. Submissions may be made anonymously, or you may provide your contact details should you desire to be contacted regarding the matter you have submitted. 

 
Process for identification, submission, review, and resolution of ethical issues.
  1. Identification of Ethical Issues: 
    • Employees may report ethical issues anonymously through the Employee Website or via the Ethical Issue Submission Form
    • Regularly, the moral team with assess potential ethical dilemmas through ethical impact assessments for new projects or policies.
  2. Formation of an Ethics Committee:
    • ​​​​​​​TRDHD Moral Team will receive and review ethical issues reported using the employee website.
    • The Moral Team has representation from different departments and levels within the organization.
  3. Issue Analysis:
    • Gather all relevant information regarding the ethical issue.
    • Identify stakeholders and consider their perspectives.
    • Evaluate the potential consequences and risks associated with different courses of action.
  4. Application of Ethical Frameworks:
    • ​​​​​​​Utilize established ethical frameworks (such as consequentialism, deontology, and virtue ethics) to analyze the situation.
    • Consider legal, regulatory, and industry standards in your analysis.
  5. Consultation:
    • ​​​​​​​Seek input from internal and external experts, including legal counsel and industry professionals.
    • Encourage open discussions among team members to gather diverse perspectives.
  6. Stakeholder Involvement:
    • ​​​​​​​Engage with relevant stakeholders to understand their concerns and perspectives.
    • Communicate transparently about the ethical issue and the decision-making process.
  7. Decision-Making:
    • ​​​​​​​Make a well-informed decision based on the analysis, ethical frameworks, and stakeholder input.
    • Document the rationale behind the decision.
  8. Implementation of Decisions:
    • ​​​​​​​Communicate the decision to all stakeholders involved.
    • Establish a plan for implementing the decision and monitor its execution.
  9. Continuous Monitoring and Evaluation:
    • ​​​​​​​Regularly review the ethical decision's impact on the organization and its stakeholders.
    • Be open to revisiting and adjusting decisions based on new information or changing circumstances.
  10. Learning and Improvement:
    • ​​​​​​​Conduct post-implementation reviews to identify lessons learned.
    • Use the experience to enhance ethical decision-making processes in the future.
  11. Training and Awareness:
    • ​​​​​​​Provide ongoing ethics training to employees to raise awareness of ethical issues and foster a culture of ethical behavior.
  12. Feedback Loop:
    • ​​​​​​​Establish mechanisms for employees to provide feedback on the ethical decision-making process.
