Age Well Santa Cruz County is an initiative of the Santa Cruz County Human Services Department, in partnership with the Seniors Council/ Area Agency on Aging and local cities. As part of the initiative, the county will be developing a Master Plan for Aging for implementing strategies and partnerships that promote healthy aging for all.
Santa Cruz County is conducting this 10 minute survey to learn more about community members’ experiences and needs. Your opinions are important! The Master Plan for Aging will be designed for residents like you.
You can take the survey if you are at least 40 years old AND live in Santa Cruz County. The survey is still open, but we are no longer offering $10 gift cards for completing a survey, as we have met our limit of the first 1,000 respondents being offered a gift card. Your feedback remains invaluable if you are willing to take 10 minutes to complete this survey – the more we learn from county residents, the more accurate our results will be.
