It Is Possible Campaign

Materials Order Form

Please use this form to request free It Is Possible materials from the Washtenaw County Health Department. Enter the quantity you are requesting in the boxes below each image.

Materials ordered can be picked up at no cost:

  • Pick up at our Environmental Health office at the Western County Service Center, 705 N. Zeeb Rd., Ann Arbor, MI 48103, between 8am - 5pm, Monday through Friday. 
  • Pick up at our Health Department office at the Washtenaw County Human Services Building, 555 Towner, Ypsilanti, MI 48198, between 8:30am - 5pm, Monday through Friday. 
If you are unable to pick up items we may be able to deliver them. 

Please note: Supplies are limited and orders will be fulfilled in order of receipt. Quantities may be adjusted based on availability. For questions, please contact Beth Ann Hamilton at hamiltone@washtenaw.org.
