Thank you for your interest in scheduling an appointment with the Public Health Madison & Dane County Sexual and Reproductive Health clinic. The following questions will help us provide you with the correct provider and appointment type.
Are you scheduling a new appointment or making changes to an existing appointment? *This question is required.
What is your main reason for coming to the clinic? If you have more than one reason, please select the most important or urgent one. We will try to address as many reasons as possible during your appointment, or help you schedule an additional appointment if needed. *This question is required.
Are you having symptoms today? Symptoms can be anything out of the ordinary for your body. Some examples of symptoms include bumps, rash, discharge, pain/discomfort. 
Do you have any of the following symptoms? If you do not see your symptoms listed, please select "Other." *This question is required.
You've told us that you're interested in emergency contraception (EC).

We have two different types of emergency contraception options at our clinic. 
1. Emergency contraceptive pills - can be used up to 3 to 5 days after unprotected sex, depending on the type of pill.
2. Intra-Uterine Device (IUD) - can be used as emergency contraception for up to 1 week after unprotected sex. IUD's may be removed at any time when no longer wanted.


 
We offer many forms of birth control including pills, patches, vaginal rings, Depo Provera shot, IUDs (hormonal and non-hormonal), and Nexplanon arm rods.
All of these options will be discussed with you at your appointment, so you are able to make the best decision for yourself.
