ASPR Getting Started - Ohio Tri-State Disaster Preparedness Coalition

Thank you for choosing to be part of the Health Equity Task Force. We would like to know a few things about you as we get started on this project. Please take a few minutes to help get to know the Task Force members.
A Little About You
Every geographic community includes multiple community sectors and community groups. People can be members of multiple groups and communities based on their personal and professional roles. Task force members may have joined the task force because of one specific role (I.e., your position at work, your participation in an advocacy committee etc.) You may also be a member of other groups/communities that should be represented in the task force. When considering your contributions to the task force please identify any population groups that you belong to AND that you would like to represent on the task force by providing input on behalf of that community group. Checking the box for community groups that you would like to represent will help us to insure diverse representation in your task force.

Please Note: The first question refers to community demographic groups. The second question refers to your professional work. The third question refers to the agency or organization you represent.
