This form will allow North Dakota's SOR grantees to enter data about monthly progress related to the grant. The data will be used by ND’s Behavioral Health Division (BHD) to monitor project progress and provide technical assistance as needed. Please enter your data by the 10th day of the following month for data from the previous month. For example, you would enter your data for September 2023 by October 10, 2023.
After you complete the Monthly Reporting Form for any Reporting Period, you will be directed to a page that allows you to review your entries. You then must hit SUBMIT for the Form to be officially submitted.
