This form will allow the North Dakota State University’s ONE Program to enter data about monthly progress related to the ND SOR grant. The data will be used by the Behavioral Health Division (BHD) to monitor project progress and provide technical assistance as needed. Please enter your data by the 10th day of the following month. For example, to report on your data from September 2023, you would enter that data by October 10, 2023.
After you complete the Monthly Reporting Form for any Reporting Period, you will be directed to a page that allows you to review your entries. You then must hit SUBMIT for the Form to be officially submitted.
