Self-Directed Supports Network - Listserv Sign Up Form

Thank you for your interest in joining the listserv for the Self-Directed Supports Network. The purpose of the list is to provide individuals and families the opportunity to ask questions, share ideas and find resources. 

Members of this listserv include adults with disabilities using Self-Directed Supports (SDS) through Rhode Island's Division of Developmental Disabilities and their family members.  As appropriate, individuals considering Self-Directed Supports may be permitted to join the list. A limited number of professionals connected to Self-Directed Supports may also be approved to participate.

Please fill out the form below to submit your request. The listserv is coordinated by the Paul V. Sherlock Center on Disabilities. Requests to join the list will be reviewed and approved by Crystal Cerullo, Family Support Specialist.

Individuals approved to join the listerv will receive a welcome letter that includes expectations as a list member and directions for posting and responding to messages.

DEMOGRAPHICSThe Sherlock Center collects demographic data to include in state/federal reports and grant applications. The Sherlock Center reports data in a non-identifying manner. Participation is appreciated but voluntary.
