Welcome to our survey on the knowledge needs of people with disabilities in the United States. It is part of a study to help us learn more about the ways you access and use information in your daily life. We will share what we learn with people with disabilities, their family members, researchers, and allies in the disability community.
The Center on Knowledge Translation for Disability and Rehabilitation Research (Center on KTDRR) is hosting this survey. The Center on KTDRR is supported by the National Institute on Disability, Independent Living and Rehabilitation Research.
We will give a $100 gift card to five people who complete the survey. We will pick them at random. To be eligible to receive a gift card, you must provide your name and contact information in the form that appears after you submit this survey.
There are no known risks for taking part in the study. The survey is confidential. We will not identify people by name when we talk or write about this study. The survey will take about 15 minutes to complete. Your participation in this survey is voluntary. You may close out of the survey at any time. If you choose not to participate, we will NOT penalize you in any way. In rare instances, a study must undergo an audit or a program evaluation by an agency that monitors us. An example is the Office for Human Research Protections. If such an audit took place, this would lead to the disclosure of your data and any other information that the researcher collects but only to the people who are involved in that audit.
