Distinguished Service Nomination Form

Thank you for participating in the Distinguished Service Award (DSA) process! Please take the time to read the instructions below; we feel it will go a long way in helping you understand the kind of information the board of directors is looking for, 

When drafting your nomination below, please don't get caught up in specific numbers of reviews by the nominee or years they have spent on committees or the board. NAACLS Staff will gather all of that data. What we would like from you, as a nominator, is your personal experience with the nominee. Measurable components like the number of reviews and years served are important to the board of directors as they select a recipient, but so are first-hand accounts of this volunteer in action. There is a distinct chance that no one on the board had the pleasure to serve with your nominee. Please take the time to help us get to know this person's service through your memories and first-hand accounts. That kind of information is incredibly valuable. We prefer not to accept nominations via email. 

Please share this survey or the NAACLS News article on social media, or anywhere you feel that former volunteers may be. We want as many nominees as possible! 

NAACLS will not notify any nominees of their nomination. NAACLS will only inform the final recipient of the award. NAACLS will award this recognition annually and keep all nominations for consideration for three years.
