PECO Application Phase 2 (post-installation) - UPDATED 10/16/2023

Introduction

PECO's EVsmart Charging Program Rebate Redemption Request
Thank you for submitting the required details and documentation needed to process your rebate redemption request.

Please have the following information and files available to complete this application (acceptable file formats for uploading: png, gif, jpg, jpeg, doc, xls, docx, xlsx, pdf):
  • Completed Rebate Reservation Request & Rebate Approval Email (sent via email to person/party who submitted the application)
  • Completed Cost Submissions Template
  • Completed Equipment Details Template
  • Photo(s) of the EV charger installation site showing all the chargers installed as part of the project
  • Rebate Recipient Template (if rebate is to be paid to someone other than the PECO customer)
In addition, applicants will be required to submit the following information after this initial application is submitted. Further instructions will be sent after receipt of this application with details on how to submit these forms.
  • Proof of total project costs and make-ready costs (e.g., invoices)
  • IRS Form W-9 (if incentive recipient is not a corporation or government entity)
If you have multiple pictures to upload, please merge them into one PDF file before uploading.

At any time, you may pause your rebate redemption request by clicking “Save and continue later” at the top of the page. You will then receive an email with a new link to return you to where you left off. NOTE: If you close your browser before your rebate redemption is complete, and don't use the “Save and continue later” feature, your responses will be lost.
