Emergency Preparedness and Resiliency Survey for Alameda County

Thank you for agreeing to participate in this survey. We appreciate your commitment.

The World Institute on Disability (WID) is a lead agency in global disability inclusion in disasters. We have partnered with The Center for Independent Living (CIL) to support disability-inclusive emergency preparedness in Alameda County. It is important that we understand the experiences and perspectives of people with disabilities in the County and what things can improve the experiences of people with disabilities during a disaster.

We would appreciate it if you took a few minutes to complete this survey.  We are interested in your honest opinion and there are no right or wrong answers. Names are not being collected and we won’t share any of your information with anyone.

If you need assistance or accommodations to complete the survey, please email DisasterHelp@centerforindependentliving.org with your request.

If there is additional information that you would like to share, please watch for upcoming focus groups or listening sessions.

*Please note: your answers to all survey questions should be regarding experiences in Alameda County only.
