2023-24 Section One School Health Services Report School Health Services Survey and Program Summary
Please use the Section One worksheet provided on the School Health Services resource page to gather data regarding your LEA and charter school prior to beginning the report survey.

Section One Report is due no later than December 1st. Section One data and position information should only be included for K-12. Please refer to Section One Definitions and Directions when noted to assure accuracy of responses.

Revisions are due no later than June 20th. See Definitions and Directions (D & D) page 1. Section One revisions should only be submitted if you have a change in total staffing positions and/or hours. This applies to school nurses who serve school children within the school health program full-time or part-time. Please include lead nurses and supervisors who also provide direct services to students. ONLY updates to these questions require a revision to be submitted.
