Explanation and Purpose of Survey
You are being asked to participate in the Brooklyn Public Library (BPL) 2023 Library Card Access Survey, coordinated by Avenue M Group. BPL is leading this research project to collect and understand policies and procedures around cardholder signup and access in public libraries across the United States. BPL will share the results of this research with the library community in early 2024.
Description of Procedures
As a participant in this study, you will be asked to complete a survey. We anticipate it will take approximately 25 minutes of your time. The survey will ask you questions about your demographics, including the country where you live and the state/territory where you work, the name of the library at which you work, how long you’ve worked there, and your role/responsibilities.
Additionally, the survey will ask a range of questions surrounding the topic of library card access, including types of information collected, requirements surrounding proof of residency and identification, probationary periods, expiration policy and subsequent renewal, cost, age requirements, and more.
Similarly, we are interested in how your library has made changes to its policies and procedures, what prompts them, and who’s responsible for approving any changes to cardholder policies and procedures.
This survey is being conducted by Avenue M Group, an independent market research firm working with BPL to produce this study.
Participation and Benefits
Participation in this study is voluntary. Upon completion of the survey, you may provide your name and email address to enter a drawing for a chance to win a $25 gift card. One hundred (100) gift cards will be issued. Individuals must be 18 years or older at the time that the survey response is received in order to be eligible to receive a prize. No prizes will be sent to individuals prohibited by law from receiving such gifts. This contact information will not be linked to your survey responses, nor will it be transferred or sold to any other parties not associated with the production of this study; it will be used solely for the purpose of the drawing.
Data Privacy
This survey is being conducted by Avenue M Group, an independent market research firm working with BPL to produce this study. Your personal data (e.g., name, email address) will remain confidential and will not be transferred or sold to other parties not associated with the production of this study. BPL staff may reach out to the library entity in order to clarify information you submit in this survey.
The name and location of the library are very important for data analysis purposes and may be included in reports created by Brooklyn Public Library and Avenue M Group based on this survey data. Your personal data will remain confidential. Please note, BPL may share the raw data (without your personal data) from this survey, including library names, with other organizations by request, including but not limited to the Institute of Museum and Library Services (IMLS), Public Library Association (PLA), Association for Rural and Small Libraries (ARSL), and Urban Libraries Council (ULC). BPL and Avenue M Group are not responsible for any reports created or published by other organizations using the raw data from this survey.
Data will be collected, transferred, and stored in the United States by Avenue M Group prior to processing and analysis. It will be stored in a secure cloud service, using multiple means of encryption, pseudonymization, and protection. Any devices used to store or process these data will be secured with full disk encryption and two-factor authentication.

We may contact you about your status of completion of the survey. These communications are limited to participation in this survey and will only occur if you have not completed it. If you voluntarily unsubscribe from communications or if you revoke consent for us to collect your information, you will not be contacted.
