Thank you for completing the Health Assessment Report and Immunization Report. Your response is very important to us.

You will receive an email to confirm the completion of your report. The email will include a PDF copy of your report. You will need to save a copy for your record of completion.

Please note that during busy times, it may take the system's server several hours to process the confirmation report that is sent to your email address. Please also note that occasionally the reports are sent to the spam/junk folder, so please check these folders in additon to your inbox.

If you do not receive an email confirmation with a copy of your report, and more than 24 hours have passed since you submitted your report, or if you need to revise your report, please do not fill out the report again.
