IVGID Magazine Mailing Opt-Out Form

Please fill out this form if you wish to opt-out of receiving a printed copy of the IVGID Magazine in the mail. Please have a copy of one of the IVGID Magazines you received in the mail handy as you will need to provide the address information exactly as it appears on the mailing label. We will also need your name and IVGID Recreation Pass number (or the name and number of the head of your household in IVGID's database at the Recreation Center).

Please format in ALL CAPS and write PO Boxes as "PO BOX ????"

Format for boxes at mail shops should be similar to "774 MAYS BLVD #10-XXX" or "930 TAHOE BLVD #802-XXX"
